All students travelling on a school bus must complete this form so that a bus
pass can be issued. The bus pass is issued to ELIGIBLE school children ONLY.
This form can be printed, completed and returned to the bus driver or to
Whitsunday Transit's office.

New Students to the area, students whose address has changed and students
in Years 1, 8 & 12 (who have repeated) must also complete the Queensland
Transport "Bus Travel Assistance Application Form".

Bus passes will only be issued to eligible students. Students not eligible
for a bus pass MUST pay the driver.

Bus passes must be carried at all times when travelling on school services or
when travelling on student fares. Bus passes MUST be produced upon request
to the bus driver. Replacement passes will incur a fee.
